A spiritual successor to Theme Hospital, Two Point Hospital is a humorous management sim with players designing, expanding, and managing a series of hospitals.

Two Point Hospital's primary gameplay is the career mode, which features over a dozen hospital levels. Each level features a different building layout, disease problems, and objectives. Some hospitals may value aesthetics and attractiveness, while others are in a cold climate and require heat management. Players must manage their funds while building the correct facilities, hiring staff, and managing the constant influx of six people.
The game lacks named characters or a central plot, favoring instead pure gameplay with high amounts of cheeky British humor (think Aardman Animations of Wallace and Gromit, or Chicken Run). Patients come in sick and sometimes die if not properly cared for, but their diseases are all puns and played for laughs, such as light-headedness manifesting as a literal lightbulb for a head.
Though humorous, Two Point Hospital is still a management sim, and based on how real hospitals and clinics operate. Patients first need a proper diagnosis before receiving the appropriate treatment. Staff must be properly trained in their fields, and get better over time (while also demanding more pay).
Two Point Hospital does not have difficulty levels. Each hospital in career mode has up to three stars that can be earned, though it's not necessary to earn all three to unlock the next hospital. The first few hospital levels are quite easy and designed as a tutorial to teach the basic gameplay concepts, while later hospitals are more challenging.
Research progress is also shared amongst all hospitals. Players can return to previous hospitals with more advanced research to obtain higher star reputations.
Scary Imagery Some mild blood is shown with sick patients. Patients can die if untreated (or misdiagnosed) and turn into ghosts, which must be vacuumed up by trained janitors.
Consumerism Two Point Hospital includes several downloadable content packs that add new regions with new hospitals, challenges, and diseases.
Two Point Hospital is a single player game. However, it does include online leaderboards through competitive challenges in each hospital level.
